If I'm gonna throw out an old cell phone, what's a responsible way to dispose of it? And what should I do to it to make sure no one can steal my personal data from it somehow?

It depends on your carrier and where you live. If you're with Telus Mobility, you can take you old phone back to the store:

If you're not with Telus, but live in Alberta, you can find out here:

http://www.albertaRecycling.ca

Deleting your personal data should be sufficient.
