Are login boxes always simple and plain? Or has anyone done anything bold or decorative with their website's login box?

I don't really see any need to decorate the login box. As long as they're easy to find and it's easy to see what you've typed, you're all set.

I'd say that there's a healthy medium between style and functionality. I've always liked the Wordpress login box, as available below:

It's not integrated on a page, rather a page of it's own. I like the modern layout, the text boxes along with the font are really nice.

I like pop-up login box. A "login" link that, when clicked, opens a popup login box.
